Early Childhood Program Packet for our Toddler, Preschool and Pre-kindergarten programs. 
Early Childhood Program children must have a physical dated within the last 12 months and a current list of immunizations prior to the child's first day at GraceWorks.

Does your child have a chronic health condition such as asthma? Does your child require medication during the time he/she is in daycare, Or does your child carry any medication in his/her backpack?  If the answer to any of these quesions is yes, please go to the Medical Information page and fill out the appropriate forms.

After-school form for our school-age after-school program.   This form is also used for school vacation weeks and for school-age drop-in students.
GraceWorks does not require either a recent physical or a list of immunizations for school-age children.  The public school system documents this information.
